SPCAA’s Ryan White Part B HIV Services Program, Project CHAMPS, will allow you to blend the mission, vision and core values of the entire organization within the context of HIV medical and social support services along with your knowledge, experience and abilities in an effort to provide the best possible care and services to eligible individuals living with HIV within the 15 county Lubbock HIV Service Delivery Area.
SPCAA Project CHAMPS Lubbock is a Ryan White Part B HIV medical and social support services provider as well as a HOPWA (Housing Opportunities for Persons with AIDS) housing provider. This is a full-time hourly position reporting to the Project CHAMPS Program Director. Regular work hours are 8:00 am to 5:00 pm Monday through Friday. Some weekend work, and work-related travel will be required on occasion, but advance notice will be provided. Must be able to adapt to flexible work schedules as established by management and to meet the needs of the clients our program serves.
The Medical Case Manager provides coordination of HIV medical care for eligible clients for various treatments and referrals including labs, screenings, medical specialist appointments, mental health, oral health care, and substance abuse treatment. The Medical Case Manager provides treatment adherence services to eligible individuals living with HIV that includes the provision of counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments. The Medical Case Manager acts as part of a multidisciplinary medical care team, with a specific role of assisting clients in following their medical treatment plan and assisting in the coordination and follow-up of the client's medical care between multiple providers when necessary. Works in conjunction with Project CHAMPS Case Managers and other Program Staff and contracted physicians to provide daily coordination of all services and assists clients in accessing other needed health and social services, primary healthcare, and specialized medical services for Project CHAMPS Lubbock clients.
Minimum educational requirements are a bachelor's Degree in Social Work, Sociology, Psychology, Public Health, Public Administration, Human Services, or another related field. Experience and knowledge in HIV/AIDS disease process and treatment and the Ryan White HIV Program is preferred. Minimum of 2 years relevant case management experience is required. Relevant Practicum or Internship experience related to fulfilling the requirements of a bachelor's OR Master's Program may be substituted for actual case management experience. Excellent interpersonal, communication, critical thinking skills and organizational skills required. Ability to communicate effectively and process information at a rapid pace to formulate and execute a plan of action with all levels of clients, employees and management, both orally and in writing using correct business English, including spelling, grammar, and punctuation is required. Ability to also effectively present information and respond to questions in Spanish is preferred.
